Submitting a Case for Consultation

To submit a pathology consultation request, please provide:

  • Completed Pathology Consultation Request Form
  • Relevant pathology reports
  • Clinical history and supporting documentation
  • Slides and/or paraffin blocks as applicable
  • Results from prior ancillary or molecular testing, if available.
